How Healthy Blue Living works

You automatically receive enhanced benefits with lower copayments and deductibles for the first 90 days of coverage. To continue receiving enhanced benefits, you and your spouse need to:

Adopt a healthy lifestyle by focusing on six high-impact health measures. Blue Care Network positions you to succeed.
Complete a Health Risk Appraisal.
Meet with your primary care physicians to complete a Qualification Form (36K PDF), which needs to be signed by the doctor. Mail this back to Blue Care Network within 90 days of enrollment.
